Packaged Plans

Combo Packs

Pick Any TWO service combinations for just $180.

You must have already had a Nutrition Assessment done before purchasing a combo pack. These can be purchased at the time of first Assessment, with an existing package or as a separate personalized package to suit your needs.

 

Payment Expected:
Payment is expected at time of first visit, and with each session depending on the package chosen. Payment can be made by cash or cheque.

Cancellation or Rescheduling Policy:
To cancel or reschedule appointments, a twenty-four (24) hour notice is required. A Registered Dietitian’s time is valuable and a prior notice will allow us to seek replacement for your time slot. If a 24-hour notice is not provided, a fee of $30.00 will be charged to your account. We appreciate your understanding in this matter.

Insurance Coverage:
For Canadians, OHIP does not cover the fees for Registered Dietitian at this time, however many extended health plans do. Some companies offer Employee Assistance Program (EAP) which cover certain nutrition services, please contact your companies EAP policy or your insurance plan to see if a Registered Dietitian or Nutritionist service is covered. We do not direct bill to insurance companies but will provide you with a detailed receipt that includes description of service, dates of counselling sessions and consulting fee breakdown.

Dietitian services are income tax deductible:
Registered Dietitians in Ontario are classified as “Authorized Medical Practitioner.” Retain your receipt and you may be able to claim dietitian fees as non-refundable tax credit for medical expense, from the government. Visit Canada Revenue Agency for details.
