To Get Started:

 

Step 1: To ensure the safety of your health information and your Medical Nutrition care, we require that you read and agree to the Consent & Waiver (please download and save this document for your records). This is a required step for booking your Nutrigenomics test. By proceeding with the next steps you acknowledge that you have read and agreed with the above.

 


 

Step 2: Complete and Submit the fields below. You will receive an email to verify your Email Address – Click on “Confirm My Request” – this step is important.

Step 4: You will receive your Nutrigenomix DNA sample collection kit to the address you provided above (in about a week). Please follow the steps and drop your sample in the mailbox using the given return envelope.

 


Step 5: You will also receive an email with a Dietary Intake Assessment (FFQ) which will take about 20 minutes to complete. This is essential in order to get the most out of your DNA report. If you do not receive an email within 48 hours, check your spam. Please add “nutrigenomix.com” and “modestnutrition.com” to your safe list to receive further communication from us.

 


Step 6: Once your report is ready (usually within 3 weeks after you mail your sample) we will email and/or call you to notify you to book your 45-minute session with the registered dietitian to go through your results.

Our laboratory uses state-of-the-art genetic testing procedures to analyze the DNA from your saliva sample. We analyze your genetic code to determine how your genes can influence recommendations related to:

What is the difference between the Health, Sport, Fertility and Plant-Based tests?

While all tests report the same genetic markers, each test is intended for a different clientele.

  • The Health test focuses on achieving optimal health for individuals who are pursuing general well-being. For example, these individuals may be interested in weight management, and improvements in cardiometabolic health and fitness.
  • The Sport test is designed to provide nutrition and performance-related recommendations that will help active individuals and athletes boost physical and mental performance while optimizing body composition and nutritional status.
  • The Fertility test is designed to provide nutrition recommendations that will help individuals (male or female) who are trying to conceive by optimizing their fertility potential.

FAQ:

Do I need a Doctor’s Referral?

To book a nutrition consultation a doctor’s referral is not required but encouraged. Please contact your physician or healthcare provider for a referral by asking them to complete this Referral Form and send it to us by fax at (289) 276 7298 or email at dietitian@modestnutrition.com.

 


Where can I send my blood reports?

We really prefer to see your blood reports if you have them! However since our sessions are virtual, please remember that email and Zoom are not secure platforms – you can skip this step – your privacy is very important to us.

Do you offer virtual consultations?

Yes, all our appointments at this time are virtual, using Zoom platform.

 


Can I book directly by phone?

Yes, you can call and book with us at 289-851-7642.

Once your date & time is booked over the phone, we will send you details of payment and the assessment form by email.

 


Can I get Insurance Reimbursement?

The services of a Registered Dietitian are not covered by Ontario Health Insurance Plan (OHIP). Some private insurance carriers do cover some of the cost. Check you private insurance carrier for details. We do not do directly billing, we can provide you with a receipt with details of your visit; however it is your responsibility for submitting costs to your insurance company.

 


Can I get Tax Reimbursement?

Registered Dietitians in Ontario are classified as “Authorized Medical Practitioner.” Retain your receipt and you may be able to claim the fees as non-refundable tax credit for medical expense, from the government. Visit Canada Revenue Agency website for details.


What is your Refund Policy?

We offer 100% refund if you cancel more than 24 hours before your initial appointment for the service/package.

 

Fees are due at or before each visit and with each service, depending on the package/payment plan chosen. All service fees are non-refundable after service is rendered (regardless of the time spent – all service timings are estimated +/- 20 minutes). If you cancel your package in the middle, you may qualify for a refund of up to 50%. All banked visits must be used within a year of purchase, unless previously arranged.

 

We do NOT issue a refund under these conditions:

♦  After you have completed your appointment and the services are rendered

♦  If you cancel or reschedule within less than 24-hours of your appointment

 


Cancellation, No Shows & Rescheduling Policy:

If you wish to cancel or reschedule, we require a 24-hours notice prior to your scheduled appointment. It is your responsibility to book and keep your appointments. All no shows will be regarded as a completed visit! (and will not be refunded).
